The present study provides a set of new data aimed at analysing the relationship between several markers of bone activity and dialysis of phosphate. A main outcome of the study was the finding of an association of some of these markers with phosphate removal by haemodialysis. However, a critical fact was that the role of these molecules is subordinated to that of the main determinant of phosphate elimination, namely, plasma phosphate concentration. To the best of our knowledge, the present study is the first to examine the potential role of bone activity and related molecules on dialytic phosphate clearance. A second outcome of the study was that none of the markers analysed appears to individually define the conditions of phosphate elimination. In fact, bone activity and phosphate removal are more efficiently described by a combination of factors. This finding suggests that there is an interaction, either stimulatory or inhibitory, which is actually relevant to determine the actual phosphorus elimination. The available reports on phosphate kinetics in HD, still do not fully explain the basis of phosphate turnover. In our study, positive predictive factors of phosphate removal were serum phosphate and PTHi OPG ratio, albeit in unequal proportions. As mentioned, serum phosphate has been found to be the critical factor in determining phosphate removal. In this regard, the relationship between pre-HD serum phosphate and the total amount of phosphate dialysed is readily understandable and our results fully confirm this point. Accordingly, the finding of higher phosphate removal in groups with higher serum phosphate, e.g. younger people or individuals with lower bicarbonate or lower OPG, was the predictable outcome. Another example of interest is the absence of significant differences in phosphate removal between the groups with extreme patterns of high and low bone turnover. The reason that upholds this finding, which apparently denies the importance of bone turnover in phosphate elimination, with the role of PTHi OPG, which supports the significance of bone activity-related factors, could be the much higher relative weight of pre-dialysis plasma phosphate in determining phosphate removal see also next paragraph ; . Moreover, the small number of patients in each high and low turnover group precludes a more definitive conclusion on this particular aspect. Nonetheless, multiple regression analysis indicates that factors other than serum phosphate have also influence, and should be taken into account in depicting the full scenario of phosphate turnover regulation. The ratio PTHi OPG was used as representative of the balance between bone resorption and formation stimuli. In our mathematical model, the PTHi OPG quotient was able to explain phosphate elimination independently; for this reason, additional attention was focused on this ratio Table 4 ; . The results obtained indicate that PTHi OPG ratio is positively related.
Strategy adopted to obtain this behaviour is described as follows: i ; where the gradient information is unidirectional, the viewer should be directed to move parallel to the image gradient, thus providing the best measurement and maximal information; ii ; in the opposite case where the aperture problem is negligible, the choice of the motion is less important, as, ideally, any motion should provide an equivalent increase in information; and iii ; when no intensity information is available, the point should be ignored, as it provides no contribution to the solution and is completely dependent on the interpolation process. These characteristics are fully encompassed by the eigenvalues, 1 and 2 , of the Normal matrix 2.7 ; , where 1 2 . Table 4.1 provides an intuitive association of eigenvalues to the three conditions mentioned above.

This paper presents the background to the development of the implementation plan, objectives of the plan, strategies that will be used to implement the activities in the plan. It also presents activities implemented so far, sources of funds to implement these activities, challenges, lessons leamt and the way forward. 15006 ORAL ; THE DISTRICT HIV AIDS GRANTS PROGRAMME IN AIM Eretu Godfrey, David Mabirizi, Denis Ahairwe BACKGROUND: The revised NSF for HP AIDS activities in Uganda repeatedly highlights resource constraints as major impediment in the provision of HIV AIDS services in Uganda. Uganda is facing a crisis in care and the demand for treatment and palliative care services by patients with HIV AIDS. This demand is expected to farther increase as these services become more available. Patients with HIV AIDS related illnesses occupied more than 55% of the Hospital beds in 1997 and by 2000 the occupancy rate had increased to 70%. PNFPs NGOs ; are faced with funding and other capacity and material constraints in providing community and home based care. The limited number of agencies providing psychosocial support are understaffed and under funded to meet the growing demand for these services.' It also emerges that the planning, resource mobilization and resource management capacity in NGOs and FBOs is very weak and yet these are vital actors in the fight against HIV AIDS. Grants have been channeled to local government and district based organizations by different mechanisms depending on the funding agency or donor. The binding agencies at the district level include bilateral and multilateral partners. AIM supports HIV AIDS service delivery in 16 districts of Uganda and includes a grants programme, that works along the UAC District level guidelines that recommend the formation of the District AIDS Committees to coordinate activities at the decentralized level. The DAC appointed Focal Persons are key in guiding and coordinating the grants mechanisms at the district level. DESCRIPTION: The AIM programme is designed to achieve the objectives through technical assistance and grants extended to government services, NGOs, CBOs and FBOs. hi order to achieve this goal AIM works with partners at the district level, in particular the District HIV AIDS Committees DACs ; to assist them in determining unmet service delivery needs gaps ; by working with local planning bodies and identify programmatic approaches by which to address them. The Grants program began in January 2003 and to date funded 251 sub-projects grantees ; . The funding ranges between USD $ 2, 500 to $ 150, 000 witfi an average of USD $ 20, 000. 30% of the grants were extended to Government agencies Government both at local and central levels ; and 70% to Non-Government Agencies 30% Community Based organizations, 10% Faith Based Organizations, 7% Non- Government Oryanizations md3%PrivateService Orsani. EXPERIENCES: The targeted solicitation of proposals process resulted in 587 proposals received by the 16 DACs. 5% of die proposals received and reviewed by the DACs were rejected on the basis of appropriateness, technical soundness and capacity to implement programmes. 38 % of the proposals were rejected by a technical review team consisting of a DAC member. The output of the process was a funding approval rate of 27% 157 ; . Of the 251 grants, 75 programmes have Home based care, 75-OVC, 16-Capacity Development to the DACs, 15-for Opportunistic infections, 20-TB, 31 grants for VCT clinical grants to the DDHS cover a number of sites supported ; . On-going technical assistance financial and programmatic ; to all grantees was carried out by the regional teams and headquarters managers over a two-year period. As a result, the 4 months period absorption capacity of funds on account liquidation rate ; for the 3 months planned activities increased by 25% from 42.1% in December 2003 Nothern-25%, Western-69.5% and Eastern-31.9% ; to 52.6% in December 2004 Nothem-51.3%. Western-59.5% and Eastern-47.1% ; . This funds absorption rate is however still low for effective implementation of activities to people in dire need of services. CONCLUSION: It is important to note that ongoing technical support can improve the implementation of activities and die absorption capacity of grantees. Mechanisms to improve appropriate absorption of funds should be designed to ensure continuity in HIV AIDS service delivery at facility and community level. Donor and Government impediments to utilization of funds markedly affect implementation of activities. They should therefore be regularly reviewed and minimized. This grants programme has a locus on local government involvement DAC ; through the focal persons. By strengthening the existing structures, ownership, supervision and implementation are ensured by the local authorities. DISCUSSION Glucocorticoid action in vertebrates is highly plastic, allowing for specificity in the response depending on life history, reproductive stage, body condition, or time of day. However, the mechanisms and functional consequences of this plasticity are still relatively unknown. We investigated CORT release, CBG levels and corticosteroid receptors in three populations of white-crowned sparrows breeding under different temporal constraints. Our data demonstrate that similar to previous studies 4, 31 ; baseline and stress-induced total CORT levels during nesting were remarkably similar in white-crowned sparrows from three different populations. However, CBG capacity and affinity differed; capacity was highest in gambelii and lower in oriantha and pugetensis.
