Patients responded rapidly with a fall in blood glucose levels from 456 t 12 mg dL at baseline to 202 -C 10 mg dL at 1 week to 120 2 5 mg dL at 4 months. Glycated hemoglobin levels fell from 18.1 t 0.4% at baseline to 8.1 5 0.2% P i 0.0001 ; at 4 months. Most patients had symptomatic improvement within 3 days. Seventeen patients had positive small or larger ; urinary ketones, and 5 had positive serum ketones. Insulin to glucose and C peptide to glucose ratios improved maximally at 1 week, indicating rapid reversal of glucotoxicity on the p-cell. At 4 months, 11 patients were receiving diet therapy alone, 29 were receiving submaximal dose sulfonylurea agent therapy, 6 remained on maximal dose sulfonylurea agent therapy, and only 3 were receiving insulin therapy. Six patients were lost to follow-up. No patient experienced hypoglycemia in the first 2 weeks of treatment. Maximal dose sulfonylurea agent therapy is a safe and effective method for treating patients with markedly symptomatic type 2 diabetes. Patients improve rapidly and are able to avoid the immediate institution of long term insulin therapy. J Clin Endocrinol Metab 81: 2423-2427, 1996.
Although the incidence of cervical cancer has fallen dramatically in the US, the trend has reversed since the mid-1990s. AfricanAmerican women have also experienced a decline in incidence, but still suffer a 72% excess in incidence and 13% lower five-year survival rate compared with Caucasians.23 Another striking difference between racial groups is the continued rise in age-specific incidence for cervical cancer in AfricanAmericans. While Caucasian women demonstrate an incidence that plateaus around 15 per 100, 000 starting at age 45, the incidence in African-American women steadily rises to about 50 per 100, 000 by age 85.23 There are marked geographical variations in US mortality rates for cervical cancer. The Midwest, South, and particularly Appalachia, have the highest mortality rates, which may be attributed, in part, to rural and socioeconomic factors related to access to health care Fig. 1 ; .23.
Abundance the goods needed to satisfy our desires, or if benevolence were to lead each person to regard her fellows' concerns as her own, there would be no free-riders or parasites to be restrained by the visible hand of co-operation. All would seek naturally to co-ordinate their actions for the common good, without putting forward opposed claims to the fruits of their endeavours, which justice must resolve. But scarcity and self-bias are not sufficient warrant for cooperative interaction. Were the scarcity faced by each person not aggravated by the presence of her fellows, then however self-biased she might be, her activities would bear little relation to those of others, and neither conflict nor co-operation would result. And could this scarcity not be alleviated by joint activities, then the domain of justice would extend only to the avoidance of mutually destructive conflicts, and not to the co-operative provision of mutual benefits. Thus among the circumstances necessary to justice we must include the variability of scarcity. The sources of satisfaction and dissatisfaction are not in fixed supply, so that by appropriate interaction overall costs may be lessened, and overall benefits increased. Yet strictly speaking, it is not the fact of variable supply, but awareness of the fact, which is important. This awareness has both its negative and positive effects. On the one hand we become aware of each other as competitors for scarce goods, and this awareness exacerbates our competition, increasing our costs. It is to this awareness that we refer when we say that reason adds to the disadvantages of nature. On the other hand we become aware of each other as potential co-operators in the production of an increased supply of goods, and this awareness enables us to realize new benefits. Thus we note also that reason offers advantages unobtainable in nature. Different theorists vary in the emphasis which they place on these aspects of our awareness. Thomas Hobbes focuses on the effect of recognizing one's fellows as competitors.' He argues that given scarcity and total mutual unconcern, each must view all other persons as in at least potential competition for the goods that she needs for survival or for greater well-being. But this creates in each person an actual preference for dominating her fellows; if she is able to establish her dominance now, then she may expect to be more.

Ened sense of life, and imaginative departure. Individuals who have such experiences seek more of them. 3 This connection is confirmed by marketing studies that demonstrate that the nature of the consumption experience--that is, the extent to which the experience is personally satisfying--is a key influence on a consumer's propensity to make repeated purchases Gobe, 2001; Schmitt, 1999; Underhill, 1999 ; . Zaltman et al. 1998 ; report that this process is also true of the arts. Kelly 1987 ; , in writing about leisure activities, claims that the quality of any activity is largely determined by the participant's level of mental and social engagement in that activity. Csikszentmihalyi 1990 ; , as noted above, describes "optimal" or "flow" experiences as those that stimulate full mental and emotional engagement--a condition requiring that the participant's skills be evenly matched to the difficulty of the task. The arts experience does not just engage the individual's emotions and intellect; it also is a social experience and often occurs in the company of others. Indeed, our discussion of intrinsic benefits highlights the importance of art as a communicative experience and the fact that social discourse about that communication can enhance the quality of the arts experience. Moreover, McCarthy and Jinnett 2001 ; discuss how the most-committed participants can become immersed in a community of aficionados who view the arts and perhaps a specific arts institution ; as an essential component of their identity. Morrison and Dalgleish 1987 ; also underscore the importance of social engagement with an arts-focused community in transforming casual to habitual participants. Stewardship is often a highly socially engaging form of participation--serving on a board, launching an arts fair, establishing a book group. If an individual does not find at least some arts experiences at least somewhat engaging--mentally, emotionally, or socially--he or she will get very little from the experiences and is not likely to become a frequent participant. Our analysis offers support for the view that frequent participants are those whose experiences engage them in multiple ways--mentally, emotionally, and socially. The more intense that engagement is, the more gratifying the experience. It is such experiences that make people into life-long participants in the arts. Indeed, Stigler and Becker 1977 ; liken the process through which an individual's growing competence in the arts increases his or her attachment to the arts experience to addiction and suggest that this process is characteristic of frequent participants. It is possible to describe an individual's level of mental and emotional engagement with a work of art on a scale that ranges from bored filling time, to relaxed and entertained, to involved, to fully engaged. It is also possible to describe social engagement along a scale from solitary, to interactive, to communal. For example, one might listen to music alone solitary ; , interact with others at intermission joint ; , or and miralax.

Driven inflammatory reaction abutting on the hair follicles might participate in the hair status anomaly. Objective: The aim of our study was to determine if ketoconazole KCZ ; which is active against the scalp microflora and shows some intrinsic anti-inflammatory activity might improve alopecia. Method: The effect of 2% KCZ shampoo was compared to that of an un medicated shampoo used in combination with or without 2% minoxidil therapy. Results: Hair density and size and proportion of anagen follicles were improved almost similarly by both KCZ and minoxidil regimens. The sebum casual level appeared to be decreased by KCZ. Con clusion: Comparative data suggest that there may be a significant action of KCZ upon the course of androgenic alopecia and that Malassezia spp. may play a role in the inflammatory reaction. The clinical significance of the results awaits further controlled study in a larger group of subjects and mitomycin.
Serum folate concentrators at each time point r 0.572 0.845; P 0.015 ; and with RBC folate concentrations at 6 months and 1 year r 0.747 0.771; P 0.001 ; . Colonic mucosal folate concentrations correlated inversely with serum homocysteine concentrations at each time point r 0.622 0.666; P 0.008 ; . Systemic measures of folate status did not correlate with colonic mucosal folate concentrations among individuals receiving supplemental folate. Our observations indicate that colonic mucosal concentrations of folate may be predicted accurately by blood measurements of folate status only among individuals not ingesting supraphysiological quantities of folate. Introduction Diminished folate status, assessed by either dietary intake or measurement of blood folate concentrations, has been associated with an increased risk of colorectal cancer and its precursor, adenoma 1, 2 ; . However, the relationship between blood concentrations of folate and colorectal cancer risk is less consistent than that observed between dietary folate intake and colorectal cancer risk 1, 2 ; . The relative insensitivity of blood measurements of folate in predicting colorectal cancer risk may be related to observations indicating that mild folate depletion, rather than the development of overt systemic folate deficiency, is a sufficient condition to enhance the risk of colorectal cancer 1, 2 ; . Also, the effect of folate on the colorectal mucosa may not be derived entirely from blood folate, because the colorectal mucosa is exposed to intestinal luminal folate that is synthesized by intestinal microflora or which has escaped small intestinal absorption 3 ; There is also some suggestion that a modest reduction in systemic folate status in subjects with colorectal neoplasia may not be apparent with conventional blood measurements of folate. Rather, a more sensitive indicator of cellular folate depletion, such as serum or plasma homocysteine 4 ; , may be necessary to demonstrate this degree of diminished folate status 5 ; . Furthermore, it has also been suggested that localized folate depletion in the colorectal mucosa, in the absence of systemic folate deficiency, may predispose the colorectal mucosa to neoplastic transformation 5 ; . Whether blood concentrations of folate accurately reflect concentrations in the colonic mucosa has been a controversial topic. Studies in which rodents were fed different dietary amounts of folate have shown that colonic mucosal folate concentrations are significantly correlated with dietary intake and blood folate concentrations 6, 7 ; . In one human study n 30; Ref. 5 ; , colonic mucosal folate concentrations measured in endoscopic biopsy samples correlated directly with serum and RBC folate concentrations r 0.62; P 0.001; and r 0.46; P 0.013, respectively ; and inversely with serum homocysteine r 0.72; P 0.001 ; , a sensitive, inverse, systemic measure of cellular folate depletion 4 ; . Remarks. The monotypic Cypressaurus hypsodontus Holman 1972 ; was described on the basis of two partial dentaries from the medial Chadronian of Saskatchewan. PTRM 2623 is referred to Cypressaurus on the basis of its high tooth crowns, a derived feature. In C. hypsodontus, the central cusp of each tooth appears higher than in PTRM 2623, but this difference could be the result of wear. Also in C. hypsodontus, the tooth crowns do not appear to taper toward the apex see Holman 1972, figure 2 ; , unlike in PTRM 2623. More and better specimens are desirable to clarify whether these differences indicate the presence of two different taxa. Scincoid MPH-1 Figure 12 ; Referred specimens. PTRM 5359 partial right dentary ; , Figure 12; 5358 right dentary fragment ; Description. Both specimens are small. PTRM 5359 is a partial right dentary with 13 full tooth positions and five relatively complete teeth Figure 12 ; . The IMS extends posteriorly to under the fifth tooth from the rear. A facet for the splenial, on the underside of the subdental shelf, extends to under the eighth tooth from the rear. There is a weak subdental shelf, but no sulcus dentalis. Two labial foramina are present, the posterior-most under the seventh tooth from the rear. Tooth replacement is intradental "iguanid" type of McDowell and Bogert 1954 ; . The teeth, as preserved, are relatively blunt especially in PTRM and mitotane.

Background: Most commercially available druginteraction screening systems have important limitations that fail to protect patients from dangerous drug combinations. We attempted to overcome the limitations of our commercial program by developing a Web-based clinical information system to serve as a safety net. This system identifies drug interactions with newly marketed medications not screened by our commercial program, and generates a second alert on dangerous interactions that were overridden during order processing. Methods: The Web-based system uses patient-specific and molindone.
CAPTOPRIL AND RENAL FAILURE Mason and Hilton Blood pressure control remained difficult despite trials of atenolol 300 mg day ; , sotalol 480 mg day ; , and debrisoquine 40 mg day ; while continuing maximum doses of hydralazine and prazosin. Bilateral nephrectomy of native kidneys was performed on October 14, 1981, without improvement. Because of the extreme difficulty in controlling the blood pressure, a further trial of captopril at a very low dosage was considered indicated. Before this, it was necessary to examine whether adequate blood pressure control would inevitably result in transplant ischemia and impaired function. Good control approximately 145 85 mm Hg ; was achieved with minoxidil 5 to 10 mg day, with no impairment of renal function nor hyperkalemia fig. 2 ; . Low-dose captopril was then given on an outpatient basis starting at a dose of 1 mg day and increasing to a dose of 16 mg day over 14 days. During this time, blood pressure control was inadequate table 1 ; and renal function became progressively impaired. After 16 days, serum creatinine had doubled 1.7 mg dl ; and further captopril therapy was considered unjustified. Five days later serum creatinine had returned to normal. Shortly after, a renal transplant arteriogram demonstrated 50% stenosis at the anastomosis to the iliac artery. This was confirmed at surgery on December 15, 1981, but unfortunately an attempt at reconstruction resulted in infarction of the kidney. Histology showed widespread areas of cortical necrosis but little evidence of rejection. The artery was considerably narrowed with intimal proliferation and damage to the elastica. The patient returned to maintenance hemodialysis. She is no longer hypertensive, her blood pressure showing the usual relationship to body weight. Discussion The potential risk of induction of precipitous blood pressure reduction by inhibitors of the renin-angiotensin system in patients with high circulating renin levels is well recognized.6"9 Typically, this may arise in patients with a combination of severe renal artery stenosis and sodium depletion 10 " when blood pressure maintenance becomes almost exclusively dependent.

We believe that it is too early to predict what type of Medicare drug benefit may ultimately be enacted. While we expect pharmaceutical use to increase as a result of wider Medicare coverage, we believe that many proposals, due to greater government involvement in pricing, may reduce the branded drug industry's pricing flexibility. Generic manufacturers, on the other hand, could significantly benefit from the shift to generic products that the Medicare drug benefit would likely fuel.
